Our Huntington’s Disease (HD) Care Clinic is designed to provide patients and families with well-rounded medical care in one visit. Emily Forbes, DO, MS director of the clinic, and her team are committed to making sure you receive the best care possible.
Before you are seen in our HD Clinic, you will need to schedule an appointment with Dr. Forbes during her normal clinic times. Once you are established in our Movement Disorders clinic, you can then schedule an appointment in HD Clinic.

We encourage you to bring your care partners and families with you. These visits focus on the well-being of you and your support team.
During our HD Clinic many different specialists see you, your care partner, and family in one visit. These specialists include:
- Movement Disorders Neurologist
- Genetic Counselor
- Spiritual Counselor
- Social Worker
- Registered Nurse
Before your appointment, our Huntington’s Disease Clinic Coordinator will mail you a packet of forms. Please fill out these forms completely. When you arrive for your appointment, please give these to the medical assistant when he or she brings you to the room. This helps our team provide the highest quality of care to you, your care partner, and your family.
You remain in the same room during your whole visit. The specialists in the clinic work as a team to make sure all the patient’s and care partner’s needs are met. You see each specialist one at a time. At the end of your time together, each specialist will hand your chart and questionnaire to the next provider and discuss any relevant information with them. This method helps us make sure all your needs are met without the patient or care partner having to repeat themself to each specialist.
At the end of your visit, we’ll ask you to complete a patient survey about your appointment. This feedback is used to make changes to our clinic to better serve our patients and their care partners. You’ll also be handed an After-Visit Summary with recommendations from each visit listed.
